Bisomax Am-5 Tablet may interact with the following medications, including:

• Other blood pressure medicines (e.g., lisinopril, metoprolol): There is an increased risk of low blood pressure (hypotension) and slow heart rate (bradycardia).
• Heart rhythm medicines (e.g., amiodarone, propafenone): There is an increased risk of abnormal heart rhythms (arrhythmias) and excessive heart rate reduction.
• Antibiotics (e.g., rifampicin): Bisomax Am-5 Tablet has decreased efficacy due to the faster metabolism of bisoprolol and amlodipine when used concomitantly with antibiotics.
• Antidepressants (e.g., fluoxetine, paroxetine): There is an increased risk of low blood pressure, dizziness, and excessive heart rate slowing.
• Herbal supplements (e.g., St. John's wort): Herbal supplements may cause decreased effectiveness of Bisomax Am-5 Tablet, especially amlodipine, due to increased medicine breakdown.

Bisomax Am-5 Tablet may interact with the following food or drinks:

• Grapefruit Juice: Avoid grapefruit and grapefruit juice as they may increase the blood levels of amlodipine.
• High Salt Foods: Reduce your intake of high-salt foods as excess sodium can counteract the blood pressure-lowering effect of this medicine.

Bisomax Am-5 Tablet can interact with conditions/diseases, including:

• Patients suffering from cardiogenic shock: Worsens the condition by further lowering blood pressure and reducing heart function. 
• Severe bradycardia (very slow heartbeat): Bisoprolol can further reduce heart rate, leading to dizziness, fainting, or even heart failure.
• Second or third-degree heart block: Increases the risk of worsening heart conduction issues, possibly leading to complete heart block.
• Sick sinus syndrome: Can further impair the heart’s natural pacemaker function, worsening irregular heart rhythms.
• Untreated pheochromocytoma (adrenal gland tumour): May cause dangerously high blood pressure due to unopposed alpha-adrenergic stimulation.

Your doctor will prescribe the required daily dose of this medicine, based on your individual needs and response to the medicine. Always adhere to this prescribed dosage and avoid any changes without consulting your doctor.

Overdosing on this medicine may result in excessive urination, dehydration, and electrolyte imbalances. Should these symptoms occur, it's essential to inform your doctor. Do not resort to self-medication or exceed the prescribed dosage.
